He was predeceased by: his wife Patricia Chism; and his parents, Wilburn Chism and Virda Chism. He is survived by: his brothers, Linton Chism (Roberta), Dean Chism (Ronda) and Buddy Chism.
In lieu of flowers, memorial contributions may be made to the family to help defray expenses.
