He was predeceased by: his parents, Leonard J Augustyniak and Geraldine H Stevens Augustyniak. He is survived by: his daughters, Ashley Bishop (Scot), Brianna Wise (Evan) and Carissa Garibay (Hunter); his brothers, Paul Augustyniak and Leon Augusty; and the mother of his children Debi Eschbach.
