He was predeceased by: his parents, Allen Lloyd Self and Annie Lillian Self (Woods). He is survived by: his brother Jerry Self (Kit) of East Peoria; his half-brother Robert Self (Tricia) of Nixa, MO; his sister-in-law Connie Self of East Peoria; and his nephews, Cody Self (Ashley) of Maryville, TN and Kyle Self (Krista) of Peoria.
