He is survived by: his children, Melvin Kostalik (Laura) and Michael Kostalik; his sibling Dorothy Slenk (Chester); and his siblings-in-law, Lottie Jerzak, Berniece Jerzak, Jean Naczas (Raymond), Edward Jerzak (Olga), Eleanore, Moll, Edwin Jerzak (Alice), John Jerzak (Mary Ann) and Norbert.
