He was predeceased by: his parents, William Wayne and Glenola Elizabeth Van Dyke (Lewis). He is survived by: his wife Peggy Jean Wallace; his daughter Peggy Van Dyke of Louisville; and his grandchildren, Hayden of Louisville and Paige Pittman of Louisville.
