He was predeceased by: his parents, Aston Bud and Pauline McIntosh (Vick); and his siblings, Beverly McIntosh, Gary McIntosh and Beatrice McCaleb.

He is survived by: his wife Jennifer McIntosh of Johnston City; his stepchildren, Letha Campbell of Johnston City and Joseph Farmer of Johnston City; his granddaughter Gracelynn Beers of Johnston City; his siblings, Brenda Litton (Ronnie) of Johnston City, Bobby McIntosh of West Frankfort, Larry McIntosh of Anna and Randy McIntosh of Johnston City; his close friends, John, Jeff, Clint and Jerry; his pet Taco; and his nieces and nephews, Amber, Ashley, Christy, Nicole, Ashtyn, Whitlea, Jennifer, Kolton, Bryce and Aston.
