With heavy hearts, we announce the death of Frank J. Kozik (Huntley, Illinois), born in Chicago, Illinois, who passed away on August 13, 2018 at the age of 79. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
He was predeceased by: his parents, John Kozik and Zophia Kozik (Sczceinski); his siblings, Maryann Zmuda and Fred Kozik (Fran); and his wife Judith.
He is survived by: his children, Diane Grafer (Scott) and Daniel Kozik; and his grandchildren, Katherine (Connor) and Meghan (Sean). He is also survived by husband, father and grandfather.
In lieu of flowers memorials may be directed to the Edward Hines VA Hospital.
