He was predeceased by: his parents, Olga Pylawka and Theodore Pylawka. He is survived by: his wife Maria Pylawka (Kozak); his children, Tamara, Natalia, Yuriy (Zhaleh) and Serhiy (Nicole); and his grandchildren, Theodore and Vera.

In lieu of flowers, the family requests that donations be made in George's remembrance to Alzheimer's Association (alz.org) or Prostate Cancer Foundation (pcf.org).
