He was predeceased by: his parents, Forest Eugene and Faye Madilyne Cook (Laird); and his brother William Cook. He is survived by: his wife Rita Cook of Flora; his daughter Shannon Faulk of Myrtle Beach; his stepchildren, Damon Chepren of St. Pete, Camille Rogers (Russ) of Oak Vale, Aaron Borders (Krista) of Flora and Aprylle Bixler (Shawn) of Yulee, Florida; his grandsons, Dillon and Michael Faulk of Myrtle Beach; his stepgrandchildren, James Rogers of Oak Vale, Devon, Samantha, Raine Chepren of St. Pete, Luna Skye and William Jacob Bixler of Yulee, Florida; his close friend Renee Vandaveer; and his pet Sandy. He is also survived by four great grandchildren.
