He is survived by: his parents, David Schwartz and Meghann Schwartz (Dermott); his grandparents, Daniel Lee, Anita Schwartz and Donald Lee; his great-grandparents, Stanley, Flora Schwartz and Mary Lou Holt; his aunt Lauren Wilhite (Uncle Sean); his uncle Greg Schwartz (Aunt Tracey); his cousins, Joey, Miles, Benjamin and Everett; and his caregivers, Amanda S., Amanda T., Rosemary and Terricka.
In lieu of flowers, memorial contributions may be made to the Children's Hospital of Illinois Congenital Heart Center or Pediatric Critical Care, Heart of Illinois Down Syndrome Association, or Congregation Anshai Emeth.
