He was predeceased by: his son Darrien Cook; and his sister Carrie Cook. He is survived by: his children, Lauren Burger and Xander Lacy; and his siblings, Lesley Howard (Greg), Dawn Verci (Eric), Wendy Armstrong, Jessica Collie, Joseph Lohman and Stanley Gambill (Michelle). He is also survived by 10 nieces and nephews.
