He was predeceased by: his parents, Player G. Cook and Margaret Cook (Moore); and his former wife Patricia Wooden. He is survived by: his wife Marilyn Kempf; his children, Patrick Cook (Katie) of Dallas, Michael Cook (Tracy) of Dallas and Marilyn Kelli Cook of Dallas, Texas; his grandchildren, Madison, Matthew, Benjamin and Jillian; and his brother Robert Cook (Joyce). He is also survived by three nieces and a nephew.
