He was predeceased by: his parents, Joe Conlin and Maggie Smith Conlin. He is survived by: his daughters, Lori Poppen (Arlyn) of Loves Park, IL, Andrea Schlabach (Duane) of Monticello, IL and Amy Henderson (Greg) of Louisville, KY.; his grandchildren, Alex Poppen (Emily) of Chicago, IL, Chandler Poppen of St. Louis, MO, Maggie, Drew Schlabach of Monticello, IL, Ella and Gabe Henderson of Louisville, KY.; and his brothers, Dave Conlin (Bonnie) of Arthur, IL and Phillip Conlin (Alice) of Effingham, IL. He is also survived by nieces and nephews.

Memorials may be made in Jack's name to: St. John's Catholic Church in Arcola, Shriners Children's Hospital or Carle Hospice.
