He is survived by: his son Jared; his wife Shelly Kiser of Hagerman; his siblings, Doris Edwards of Omaha, Ron Foukal (Elaine) of Jerome, Betty McGuire of Grand Junction and Darlene Wright (Bob) of Emmett; his daughter Julie Stone (Chet); his siblings, Janelle Sobotka (CJ) and Falls (Jared Foukal); and his grandchildren, Kadence, Toby, Brayden and Reagan.

The family suggests memorial contributions be made in Dave's name to the A.L.S. Foundation.
