He was predeceased by: his parents, Lyle V. Prochaska and Mildred Evelyn Prochaska (Schackow); and his brother Dan Prochaska. He is survived by: his former wife Karen L. Wiebbecke; his daughter Megan Prochaska (Justin Klein) of Evansdale; and his siblings, John Prochaska of Waterloo and Roxzane Bamford (D. J.) of Waterloo.
