He was predeceased by: his parents, Howard J. Cook and Anna E. Cook (Speas); his daughter Carol Ann Rasmussen; and his siblings, Virginia Harrellson, Gertrude Meyers, Marie King, John "Bud" Cook and Everett Cook.

He is survived by: his wife Bonnie F. Cook; his daughters, Cathy J. Hale (Tony) and Cherie E. Cook; his grandchildren, Ryan Rasmussen (Kaidy), Renee Rasmussen, Carrie Burk and Jon Burk (Rachelle); and his brother Leslie H. Cook (Jane). He is also survived by four great grandchildren.
