He was predeceased by: his parents, Francis Joseph and Ruth Linnea Kujac (Cantrell); his grandmother Minnie; and his sister-in-law Phyllis Kujac. He is survived by: his wife Miriam Siehl of Madrid; his daughters, Amy Kujac of Wisconsin and Anne Kujac Schlieman (Mike) of Madrid; his grandchildren, Jenna Kate, Brody John and Breanna Grace Schlieman; and his brothers, Frank Kujac (Minnie) of Nevada and Robert Kujac of Des Moines.
In lieu of flowers, memorials may be made to Hopkins Grove United Methodist Church or Hospice of the Midwest Johnston.
