He is survived by: his wife Michelle; his children, Jessica of Des Moines, Jade of Mitchellville, Alex and Deklan; his grandsons, Brock Lewis and Aidan Lewis; and his brothers, Jeff Lewis (Leona) of Baxter and Justin Lewis of Newton.
In lieu of flowers, memorials may be directed to the Colfax-Mingo Youth Sports Organization, Colfax-Mingo Little League or Mercy Hospice.
