He is survived by: his wife Helon Middlebrooks; his children, Monica Cardin (Tim) of Murfreesboro, TN., Herbert Middlebrooks, Jr. (Sheila) of Murfreesboro, TN., Chris Middlebrooks (Lora) of Hazel Green, AL., Maronica Blackmon (Kevin) of Davenport, FL. and Daniel Middlebrooks (Sonya) of Trenton, GA.; his grandchildren, Brandy Cardin, LeAnne Bridges (Michael), Haley Pritchett (Bradley), Kyle Middlebrooks (Amy), Scott Middlebrooks (Tyree), Beth Owens (Cody), Andrew Middlebrooks, Kelsey Le Blackmon, Avery Blackmon (Lynzie), Brandon Middlebrooks (Katie), Emma Middlebrooks and Elisabeth Middlebrooks; his great great grandchild Middlebrooks Tina Haney; and his brothers, Troy Middlebrooks (JoAnn) and Terry Middlebrooks (Barbara). He is also survived by nieces, nephews, and a host of friends.
