He was predeceased by: his parents, John Bolthuis Sr. and Everdeena Bolthuis; his wife Mary Alice; and his brother Francis Bolthuis. He is survived by: his children, Gary Kevin Dunaway and Linda Ann Dunaway. He is also survived by 5 loving grandchildren.
