He is survived by: his parents, Jack Cutts Smith, Jr. of Moultrie and Mary Ann Moak Smith of Moultrie; his grandmother Roberta Moak of Moultrie; his nieces and nephews, Jacob Conger, Kathryn Conger, Elizabeth Conger, Luke Conger and Gigi Smith; his aunts and uncles, Rhett, Mark Moak, Owen Smith, Joan and Roger Smith; and his cousins, Leah Moak (Cody Hoover), Daniel Moak and Ellen Buer (Eric).
