He was predeceased by: his parents, Ossie Cook and Mary Anderson Cook; his son Doug Cook; and his siblings, Johnny Cook, Lavenia Cook Key and Edward Cook.
He is survived by: his wife Martha Quinn Cook of Milledgeville; his daughters, Deborah Cook of Milledgeville and Dianne Lyon of Evans; his sisters, Sadie Cook Boyer, Gloria Cook Edwards and Joyce Cook Stanton; and his grandsons, Alex Lyon and Andrew Lyon.
