He is survived by: his wife Teresa Sullivan; his children, John "Sullivan" Butler (Ashlie), Josh Cook, Tyler Cook (Ashley), Brandy Cook, Mandy Goble (Chris) and Sandy Alcorn (Robbie); his grandchildren, Aubrey, Landon, Henry, Atley, Memphis and Heidi; his stepmother Sharon Sullivan; his siblings, Kenneth Sullivan and Brandy Ogle (Matt); his nephew Gabriel Ogle; and his niece Brittany Ogle.
