He was predeceased by: his parents, Luther Jackson Underwood and Eunice Wilson Underwood. He is survived by: his wife Janet Mize Underwood; his sons, Michael Underwood (Lisa), David Underwood and Jeffrey Underwood; his grandchildren, Liam, Emily and Matthew Underwood; and his siblings-in-law, Sheila Mize, Linda, Ralph Kahlan, Lee and Alisa Mize. He is also survived by nieces and nephews.
