He was predeceased by: his parents, Samuel Levine and Theresa Levine. He is survived by: his wife Kathy Levine of Ellijay, GA.; and his siblings-in-law, Rosemary, Michael Shaver of Canton, GA., Karroll "Ky" and Randy Annunziata of Ellijay, GA..
