He was predeceased by: his father James M. Hazlett. He is survived by: his daughter Ashley Sullivan (Dylan); his mother Janet L. Hazlett (Thompson); and his siblings, Tina Jamison, Melanie Dyess (Tony), James Hazlett Jr. (Pam) and Scott Hazlett. He is also survived by nieces, nephews.
