He was predeceased by: his parents, Frank Hazuka and Melvina Hazuka; and his siblings, Orville Hazuka and Fran Hazuka. He is survived by: his wife Sally Hazuka; his sons, Gerald Hazuka (Cindy) and Gerell Hazuka (Dixie); and his grandchildren, Tonny and Sam.
