He is survived by: his children, Sherry Anderson of Decatur, Michael (Jennifer Butler) and Matthew (Katie Butler); and his grandchildren, Wesley Butler, Lauren Anderson, Josh Maxey, Brooke Butler, Emily Butler, Chase Butler, Mason Butler and Brock Butler.
In lieu of flowers, donations may be made to the American Kidney Foundation in David's memory.
