He was predeceased by: his parents, William Lee Mayfield and Martha Louise O'neal Mayfield; his former wife Willie Jo Smith Mayfield; his son-in-law Eddie Waters; and his siblings, Marlin Mayfield, Billy Joe Mayfield, Mary Lee Mayfield Thomason, Martha Irene Mayfield Holt, Essie Bell Mayfield Thomason and Donald Ray Mayfield (Barbara Jo Mayfield).
He is survived by: his wife Carolyn Mayfield; his children, Angie Mayfield Waters, Mike Pinkard, Linda Scoggins, Sharon Waddell, Janet Pastovich, Lisa James and Steve Pinkard; and his siblings, Hugh Thomas Mayfield, Sr., Willie Anderson Mayfield and Sarah Loue Mayfield Gibson. He is also survived by 19 grandchildren; 39 great grandchildren.
