He was predeceased by: his parents, Cecil Formby and Clyde Mae Formby; and his siblings, Larry Formby, Michael Formby and Tina Formby.

He is survived by: his sisters, Linda Formby and Donna Faye Formby; and his cousin Levon Ward. He is also survived by several nieces, nephews and cousins.
