He was predeceased by: his father Thomas Fazio. He is survived by: his mother Lillian Fazio (Pfautsch); his wife Teresa M. Ciorciari; his children, Anthony (Danielle), Christina (James) and Elizabeth; his brothers, Thomas (Luanne), Pete (Lydia) and Jimmy (Jen); and his grandchildren, Rosario, Maximus, Ritchie, Giovanni and Baby Boy.

In lieu of flowers the family requests donations be made to ACTS (A Community that Shares).
