He was predeceased by: his father Irvin Ellsworth. He is survived by: his mother Barbara Ellsworth; his children, Carly (Justin) and JD; and his grandson Jackson.
A memorial service will be held on Sunday, December 8th 2024 at 2:00 PM at the Trinity United Methodist (4150 S Shade Ave, Sarasota, FL 34231).
