He was predeceased by: his stepfather Albert Dvorak; and his brother Theodore Wayt. He is survived by: his mother Kathleen Dvorak; his sons, Chase, Stephen and Tristin; and his siblings, Albert, Beth, Cindy and Kathy. He is also survived by many nieces and nephews.
