He was predeceased by: his mother Ruby Dell Cook; and his grandparents, Ardell and Jewell Harrison. He is survived by: his son Richard Duane Cook (Lyndsay); his grandchildren, Jesse Nance and Cailyn Cook; his father Richard W. Cook; his sister Rochelle Henson (RJ); his nephews, Cole and Levi Mathews; his brother Randall W. Cook; his son Brandon (Casey); and his grandchild Colay.
