He was predeceased by: his parents, Hoyt Banks and Kate Banks; and his siblings, Elbert Banks, Ronald Banks, Frank Banks, Johnny Banks, Kathryn Galante and Marjorie Freeman.
He is survived by: his daughters, Deanna Smyth (Jeremy) and Brittnie Holt (Richard); his grandchildren, Jacob, Emily, Kyleigh, Brianna, Jemma, Veda and James; and his siblings, Sue Sommer, Frances Nelson (Raymond) and Morris Banks. He is also survived by many nieces, nephews and cousins.
The family asks, in lieu of flowers, that a donation be made to the Alzheimer's Association in the name of David Michael Banks.
