He was predeceased by: his son Kevin. He is survived by: his wife Suzanna; his sons, Jim, Jr., Michael (Amy) and John (Daniela); and his grandchildren, Miranda, Kaylie and Madeline.
In lieu of flowers, the family requests donations to either American Diabetes Association or Community Hospice and Palliative Care, Jacksonville, Florida.
