He was predeceased by: his parents, Fred Dively and Thelma Dively; his sons, Gregory Dively, Brian Dively and Jerry Dively; and his brother Sherman Dively.
He is survived by: his wife Theresa Dively of Interlachen; his children, Tommy Dively, Jr. of Interlachen, Terry Dively (Doris) of Pennsylvania, Michelle Dively Miller (Ron) of Pennsylvania, Michael Dively of Pennsylvania, Hamp Dively of Pennsylvania, Doug Dively (LeeAnn) of Pennsylvania, Laurie Markel of Pennsylvania and Darlene Rivera (Willie) of Keystone Heights; and his sister Maureena Dively Dubbs of Pennsylvania. He is also survived by numerous grandchildren and great-grandchildren.
