He was predeceased by: his parents, James Lewis and Annie Lewis; and his brother Steve Lewis. He is survived by: his children, Elizabeth Lewis, Anthony Lewis, Jr. (Rachel), Derek Lewis, Christian Moody and Kelsey Lewis; his siblings, Jeffery Lewis (Mary Yvonne), James Lewis, Jr. (Diana) and Terry Lewis (Donna); and his grandchildren, Madison Branham, Mason Branham, Kacey Lewis, Austin Lewis, Connor Lewis, Kaitlyn Ward, Zoey Boswell, Paradice Boswell and Chase Gilbert. He is also survived by numerous nieces, nephews and other extended family.
