He was predeceased by: his parents, Rosa Cook and Roy Cook; and his siblings, Miriam Cook, Deborah Cook, James Cook, Vivian McDonald, Darlene Cook and Stanley Cook.
He is survived by: his close friend Gay Nelle Harris; his children, Nikia Cook (Gay Nelle Harris) of Seville, FL and Dewayne Cook (Jennifer) of Daytona Beach, FL; his sister Valerie Rhodes (Ricky) of Sylvania, GA; his sisters-in-law, Belinda Cook of Sarasota, FL and Rosetta Cook of Crescent City, FL; his godchildren, Maurice Cook of Sarasota, FL and Gabbriella Burrell of Jacksonville, FL; his uncles, Albert Cook (Rochelle) of Atlanta, GA and Edmond "Perry" Cook of Atlanta, GA; his nieces, Sa'Mia and Sa'Mari Gibson of Deland, FL; and his nephew Micah Harris of Deland, FL. He is also survived by loving nieces, nephews, aunts, and friends.
