She was predeceased by: her parents, Emily Bumgart and Richard Maday; and her sister Delphine Maday. She is survived by: her husband Daniel Kozlowski; her daughters, Jaclynn and Rachel Kozlowski; and her brother Lawrence Maday. She is also survived by numerous nieces, nephews, and close family friends.
