She was predeceased by: her parents, Ambrous Olin Willingham Sr. and Alda Ruth Willingham (Miller); her husband Clarence Calvin Cook; her siblings, Olin, Earl, John, Ted Willingham, Ann Willingham, Ruth Yon and Joy Thomas; and her grandson Jason Mathers.
She is survived by: her sisters, Myra Berley and Helen Smith; her children, Carol Mathers (Keith), Sharon Spivey (Tom) and David Cook; her grandchildren, Leah Doyle (Michael), Kyler Spivey, Jessica Spivey, Ryan Mathers and Eric Mathers (Cynthia); and her great grandchildren, Moira Doyle, Declan Doyle, Otis Doyle and Serafina Mathers.
