He was predeceased by: his former wife Nancy. He is survived by: his wife Donna; his children, Timothy, Michael (Susie) and Tana (Duane); and his sister Sandy Ballard. He is also survived by his 9 grandchildren, 8 great-grandchildren, 1 soon-to-be great-great-grandchild.
