He is survived by: his siblings, Kevin, Ken and Kathy; his wife Bridgette Kostelnik; his children, Cindy Machuca, Matthew Kostelnik and Joseph Kostelnik; and his daughters-in-law, Jessica Dixon and Ashlee Kostelnik.
