He was predeceased by: his wife Sally Y. Guy. He is survived by: his children, Jane (Jassen), Tom Jr. and Erin (Jeff); his grandchildren, Spencer (Kalyn), Leah (Taylor), Zoe, Garrett and Kendall; his great grandchildren, Gabriel and Denver; and his brothers, Denny and Tim.
The family asks that in lieu of flowers, donations be sent to the Lustgarten Foundation for Pancreatic Cancer Research.
