He is survived by: his wife Cipprianna Cook; his children, Bruce Cook (Edwina) of Blue Ridge, Texas, Sherry Housley (Gene) of Rogers and Denise Good (Kerry Blalock) of Columbus, Mississippi; and his siblings, Leon Cook of Hydro, Dwight Cook of Prairie Grove, Hubert Cook of Cresswell, Roy Dale Cook of Madill, Michael Cook of Lincoln, John Wayne Cook of West Fork, Arkansas, Ruby Reed of Springdale, Joyce McCaslin of Elkins and Brenda Ackley of Gentry, Arkansas. He is also survived by seven grandchildren, eight great grandchildren and two great-great grandchildren.
