He was predeceased by: his parents, Loyd Cusick and Inez Cusick; his daughter Susan Martin; his son-in-law Randy Martin; his brothers, Jerry Cusick and Jimmy Dale Cusick; and his granddaughter-in-law April Owens.

He is survived by: his wife Mary Cusick; his daughters, Sharon Baggett (David) of Van Buren and Alica Johnston (Eddy) of Greenwood; and his grandchildren, Joel Brown (Destini), Jeremy Brown, Jordan Vail (Alicia), Brandon Martin (Amy), Jared Martin and Seth Martin. He is also survived by nine great grandchildren.
