He was predeceased by: his parents, John Mayfield and Mildred Pauline Gray Mayfield; and his brother Donnie Mayfield. He is survived by: his wife Theresa Mayfield; his children, Nate McMillian of Altus, Scotty Mayfield (Amanda) of Ozark, Justin Mayfield of Altus and Amber Mabe (Zach) of Altus; his brothers, Ricky and Billy Mayfield of Altus; and his grandchildren, Addisinn, Ryleigh, Cenyann, Trent McMillan, Ethan Mabe, Haleigh and Kynsleigh Mayfield.
