He was predeceased by: his former wife Margie Hayes Givens; his sons, Johnny Ray Givens and Scott Jason Givens; and his granddaughter Megan Leigh Cook. He is survived by: his wife Rhonda Sanders Givens; his daughters, Karen Cook (Jerry) and Bridget Ballentine (Marcus Jennings); his daughter-in-law Amanda Givens; his grandchildren, Catrina Cook (David Beck), Kelsey Moody (Dakota) and Morgan Gensel; and his great grandchildren, Brooklyn Beck, Natalie Moody and Alexander Gensel.
