He is survived by: his wife Pat; his children, Chip (Joan Kuykendall) and Karen (Keith Smith); his grandchildren, Ana (Anderson Givhan), Aaron (Paige Kuykendall) and David III (Abbey McDanal); his great grandchild Charlotte Givhan; his sister-in-law Liz Kuykendall; his niece Kathy Elrod (Johnny); and his nephew Danny Kuykendall (Meredith).
