He was predeceased by: his parents, Paul "Doc" and Hazel Azlin; and his sister-in-law Myrleen Azlin. He is survived by: his wife Jonnie Azlin; his daughter Trish Azlin; and his siblings, William Ralph Azlin (Martha), James Newton Azlin and Louise M. Hansen (Roger). He is also survived by numerous nieces and nephews.
In lieu of flowers, memorials may be made to the American Lung Association or the American Cancer Society.
